Contractor Approval Requirements

For FHA 203(k) Standard and Fannie Mae Renovation loans, eLEND must accept, prior to loan closing, all General Contractors who will perform or oversee renovation work.

Required documentation (in addition to this questionnaire):

Copy of applicable business license(s)
Principal or officer government-issued photo ID
Completed and signed W-9 form
Certificate of General Liability Insurance
Proof of Workers’ Compensation Insurance (when applicable or separate from the General Liability policy)
If exempt, provide evidence of exemption if provided by the state. If the state does not provide, provide explanation for exemption

Please release this information to eLEND to complete the validation process. It should be clearly understood that the information requested is being collected as part of a validation review process. Credit or background checks may be made at any time prior to and/or during the construction phase. I/we understand and agree that eLEND intends to use such reports for purposes of evaluating financial readiness to perform construction-related services.
